Supervising Teacher Verification Form (DOC -33 KB)
This form should be completed for eligible participants to recieve SBCEU's for participating as a supervisory teacher to a student teacher for a full placement period.
School Committee Verification Form (DOC -33 KB)
This form should be completed for eligible participants to receive SBCEU's for participation on an Approved School Committee that meets six (6) or more times per school year. Participant must sign in and out for each session.
School Improvement Team Verification Form (DOC -33 KB)
This form should be completed for eligible participants to receive SB-CEUs for participation on district or building School Improvement Team.

This application MUST be turned into the Office of State and Federal Programs at least 45 days prior to the start of the session
