Welcome to the School Nurse webpage.  Monroe Public Schools has one school nurse who coordinates health services for the school district.  This website contains a few of the many sevices she provides, as well as, some important topics for parents.  If you need to contact her she is available via email or voice mail.

WHEN IS YOUR CHILD TOO SICK FOR SCHOOL? - Please go to the following link at the bottomw of this page for information.

HEARING SCREENING - Students in kindergarten and grades 2, 4 and 6, special education students, and teacher referrals are given an audiometer test. Two audiometer tests are given before a referral is made to the parents.

VISION SCREENING - Students in grades 1, 3, 5, 7 and 9, special education students, and teacher referrals are checked for suspected vision problems.

IMMUNIZATIONS - Michigan laws state that all children in Michigan schools shall be immunized or protected against diseases specified by the Director of Public Health. Immunizations are required for DPT (diphtheria, pertussis, tetanus), polio, measles, rubella, mumps, Hepatitis B, meningococcal and chicken pox.  In case a child has not been immunized due to religious convictions or medical reasons, the parent must contact the school to apply for an exemption.

STUDENT’S EMERGENCY INFORMATION AND AUTHORIZATION FOR RELEASE - Health services are provided for students at all grade levels, pre-school through high school. Each year parents are asked to complete for our files a student’s emergency information and authorization for release form. We would like to emphasize the importance of completing it and returning it to the school as soon as possible. If your child has or had some serious physical ailment or disability, it is imperative that the school be informed. Almost every day we have a student who becomes suddenly ill in school or has an accident requiring contact with the parent or someone designated on the form. This form lists the name of your child’s physician, and someone whom we can contact when we cannot reach you at home or work.

PHYSICAL EXAMINATION - It is recommended that each child entering kindergarten have a physical examination. Parents should allow sufficient time for their child’s physical prior to the start of the school year. Physical examinations are required for students who participate in interscholastic athletics in grades 7 through 12.

STUDENTS REQUIRING MEDICATION DURING SCHOOL HOURS - School district policy is to provide adequate control and supervision of the administration of medication for any pupil who must take medication prescribed by his/her physician during school hours.  Medical authorization forms must be on file in the school office before any medication will be administered to a student. Such forms will contain the written request of the parent or guardian, and complete instructions by the physician. Forms may be obtained from the principal/secretary of the school the student is attending. Medication must come to school in the original container. No over-the-counter medications will be accepted without written instructions from the student's doctor.  See below to download the student medication form.

INSTRUCTIONAL ACTIVITIES - The school nurse is involved in the provision of instruction in the following areas:

  • Physical growth and development instruction, which covers the physical changes occurring during puberty, is presented to boys and girls in the fifth grade in all schools. Parents can request that their child not participate. 
  • HIV/AIDS instruction is presented in each school building.  Various approved videos and literature will be presented. Parents will be notified by a letter before the program. They can request that their child not participate. Videos can be viewed upon request through the school office.
